RE:                     Requests for Replacement Fuel Cards

Effective immediately, Logistical Services will be assessing a $4.00 per fuel card fee for any/all lost or misplaced fuel card(s) requests.

This fee is necessary due to the added fuel card expense from our fuel vendors.

Each operating department that requests a fuel card replacement will be asked at the time of their request to provide your department’s index number.   On a monthly basis, you will be IDT’d for this service.
