Health Insurance Coverage
All full-time students are required to have
health insurance coverage. Also, all part-time
students who participate in a clinical experience
are required to have insurance coverage. If
you are in these categories, you will be billed
by UMDNJ for coverage by Aetna, unless you complete
a waiver proving comparable coverage. Your proof
of comparable coverage must be done prior to
enrollment in classes or by the dates below
(which ever comes first). Otherwise you will
be required to purchase the Aetna policy.

If you choose to join Aetna, the insurance
coverage periods are as follows:

If students miss the deadline for on-line waiving there will be a non-refundable $100.00 fee for
students who appeal the insurance charge. This fee applies to both acceptance and rejection of the appeal.
The rates for students will be as follows:
Annual Plan (8/1/08-8/1/09) - $1,967
Winter Plan (1/1/09-8/1/09) - $1,147
Summer Plan (5/1/09-8/1/09) - $492

DeltaCare dental insurance
UMDNJ will once again offer our students, postdoctoral
fellows and their dependents the DeltaCare dental
insurance plan for AY07-09. Enrollment in this
plan is voluntary; it is not required insurance.
Students and postdoctoral fellows interested
in participating in the plan will enroll on-line,
beginning June 9th, through University Health
Plans (UHP). The on-line enrollment form, plan
benefits and highlights and a list of network
dentists can be found by linking to http://www.universityhealthplans.com/temp/UMDNJPD/intro.html, and following the links to the UMDNJ dental
insurance information.
The premiums will be:
Student Only-$254.60
Student + 1-$485.80
Student + 2 or more-$763.80
July 15, 2008 is the enrollment deadline for
coverage from August 1, 2008 to July 31, 2009
. Deadlines of August 15, 2008 and September
15, 2008 have been established as well for September
1 and October 1 effective dates respectively.
Students who enroll after the first deadline
of July 15 may not appear on their dentist's
roster until October, and their dentist will
need to call DeltaCare to verify eligibility.
The premium will not be pro-rated for the September
and October effective dates. Students with questions
regarding the plan or the enrollment process

Voluntary VSP Vision Care Plan
University Health Plans is now offering students
and their dependents a vision insurance plan,
effective September 1, 2007. This insurance
is not required and you may enroll on a voluntary
basis.
The cost for the vision insurance for

University Long-Term Disability Income Insurance
The University of Medicine and Dentistry of New Jersey, in conjunction with the American Medical Association (AMA) and Professional Benefit Consultants, Inc., will continue to provide disability income insurance tailored for our full-time students. This plan is designed to meet the particular needs and concerns of individuals who have chosen to dedicate themselves to the health professions. You are making a major investment in your future careers which can be placed at risk by the physical as well as financial effects of illness and disability.
The plan contains important features -- portability, guaranteed future insurability, and provisions such as a loan payoff, and lump sum and survivor benefits. Guaranteed portability after you graduate and leave the University, and the ability to purchase increased insurance benefits at a later date when your income has grown, without medical screening of any kind, are especially important. If you become totally disabled while you are a student and remain totally disabled, benefits are paid for life and are not offset by social security payments, worker’s compensation or other disability income insurance benefits.
The plan being offered for purchase is summarized on the accompanying attachment.
Highlights of the plan are:
- Guaranteed insurability – Professional Benefit Consultants will not deny coverage even if your health changes.
- Favorable disability definition - As a full-time student, you will be considered disabled for the first 5 years if you cannot attend professional school on a regular basis and maintain your status as a student in good standing.
- If you become totally and permanently disabled, the plan will repay eligible student debt for which you are legally liable, and college and/or medical education expenses up to $175,000.
- Benefits are payable for life with waiver of premium if you are disabled.
- Portability - You may take this insurance protection with you when you leave UMDNJ. Full-time nursing students, allied health students and graduate students in the biomedical sciences may convert to Professional Benefit Consultants’ individual disability income product upon graduation.
- Any salary or other compensation of $3,000 or less received by a disabled student will not reduce the monthly disability benefit.
If you wish to enroll in this plan for the 2008-2009 Academic Year, fill out the attached enrollment form within 30 days of the start of your school year, include a check in the amount of $93.60 payable to Standard MPA Program and mail to Professional Benefit Consultants, Inc., 7525 S.E. 24th Street, Suite 350, Mercer Island, WA 98040.
