Medical and Psychological Evaluation Services at UMDNJ
RFQ10-001
The University has contracted with four (4) vendors to provide Medical and Psychological Evaluation Services University-wide. The contract is RFQ-10-001 and is in effect through November 1, 2012, and may be extended three additional one-year periods at the discretion of the University. The table below lists vendor name, address and contact information. To request services for your department, please follow the “Method of Engagement” below or you may contact Denise Council at #5-9060 for additional information.
Method of Engagement
a. Except for emergent matters (see “e” below), the requestor will select a minimum of three Contractors for each new Medical and Psychological Evaluation Services request. The selected Contractors will be provided with specific information for the type of Medical and Psychological Evaluation Services engagement.
b. The Contractor shall provide a project plan detailing the Contractor’s approach to the requested engagement. The plan shall include an estimate of the hours required to perform the engagement, work schedule and list of individuals who will be assigned to the engagement.
c. In some instances, the Contractor may be asked to meet with a staff member in the requesting department to discuss the Medical and Psychological Evaluation Services before a Contractor is selected.
Engagement award criteria in addition to pricing will be considered as follows:
Experience in performing Medical and Psychological Evaluation Services for the particular service requested.
The approach and plans to perform the services requested.
d. Award selection for each project will be determined by the requestor’s representative, who will review and evaluate the responses and quotes received. Quotes will be evaluated based on the Contractor’s experience in the type of Medical and Psychological Evaluation Services requested and fee structure.
e. If a department/unit faces an emergent matter, then the department/unit may unilaterally select any one of the awarded Contractors. In such circumstances, the department/unit shall justify explaining the emergent circumstances in its purchase requisition.
f. A UMDNJ Purchase Order will be issued to the selected Contractor in the amount of the quote submitted as an official method of the engagement award.
