Topic : Risk Analysis v1.1

This form was created to act as a dash board or overview of several patient care issues.

  • CHD risk equivalents - are demonstrated here according to ATP3.
  • Adult Treatment Panel 3 Lipids Section - Displays number of cardiac risks or presence of coronary heart disease equivalents, last LDL (primary treatment goal of ATP3) and the goal LDL according to ATP3.
  • Joint National Committee (JNC) 6 Hypertenstion - Displays information related to the Hypertension Education program of the Hypertension Education Program of the National Institutes of Health. Note that system screens for some of the risk factors related to decision about goal blood pressure and displays its analysis.
  • Tobacco Status - Displays last signed tobacco use status (current, prev., or never). This is a major risk factor for several disease itself, but is factor in determining CHD risk under ATP3.
  • 10 Year CHD Risk (ATP3) - Displays the coronary heart disease risk for the next 10 years calculated from the Adult Treatment Panel 3 Report of the Cholesterol Education Program of the National Institutes of Health.
    • The user can update appropriate values here by entering them and then signing total document.
    • BECAREFUL to only sign off on values that are actual
