Topic : Patient Education

The Patient Education Form of the Logician system gives the provider seeing a patient at the point of care easy access document patient education.

This is the form in its neutral position. It notes that the electronic record system has been informed that language and reading are issues with education. You can click here to see the form filled out.

  • Person Educated - Names if not patient as appropriate.
  • Topics - Check all that apply and elaborate on them as needed in the details section or other parts of the notes.
  • Barriers - Check all that apply. Note that the previously checked barriers are displayed in yellow and as checked areas. The text translation appears when the check section is updated.
  • Response to Education - Check the proper response.
  • Informed How - Check all methods of informing that apply.
  • Patient Understanding - Check all that apply.
  • Details - Enter any comments on any of the above here. Note that clicking the action button will re-enter the previous value saved.
  • Language area - assists documentor with entering language preference, translator and using AT&T language line. This area will show language preference to user, but will only show it in text translation when it is updated.
