NJ Dept. of Health and Senior Services - NJ Sanitary Code (N.J.A.C. 8:57A-1.2)

A physician must be reported to Department of Health (609) 984 - 1863 or fax (609) 292 - 6577 within 30 days of diagnosis. They will guide you to special form. Included are all confirmed or clinical diagnoses that include the following words:

    • Cancer
    • Malignant
    • Carcinoma
    • Lymphoma,
    • Sarcoma
    • Leukemia

A physician must be reported to Department of Health (609) 984 - 1863 or fax (609) 292 - 6577 within 6 months of diagnosis or . They will guide you to special form.
