Provider Application Procedure

Interested candidates are encouraged to submit an LRP provider application prior to completing the last year of residency or (if applicable) professional training or dental school training, but no later than 2 years after graduation or completion of residency program.

Application packages can be obtained by contacting the LRP office by telephone or written request. Applications are accepted on a rolling basis. LRP staff will contact qualified applicants to schedule an interview. Ineligible applicants will be notified via correspondence, where the reason will be specified.
