A new access policy is being implemented at the UMDNJ - University Libraries, system wide. Effective October 1 2007, there will be no walk-in access at the UMDNJ Robert Wood Johnson Library of the Health Sciences for unaffiliated members of the public unless they first apply for access.

This form must be completed in order to apply for access.  If you are submitting your request electronically, click on the link for the campus library that you wish to visit and provide the required information.  If you want to receive approval via return e-mail, you MUST provide us with your e-mail address.  You may also choose to complete the form in writing while on-site or submit via fax to the campus library of your choice.
