*Inclement weather notifications: Please call (732) 235-9290 by 7 AM on the scheduled day of class for cancellation notification. TO REGISTER: Please Mail or Fax the SCREENER REGISTRATION FORM to the Technical Assistance Center. For timely processing, registration forms must be filled out accurately and completely. If registration is full, you will be notified. Telephone registrations will not be accepted. Only active screeners can register. Screeners must register by the deadline date, which is two weeks before the class. Late registrants and walk-ins are not guaranteed admission to classes, and may be turned away. TMobile Practicum must be arranged by calling TAC at least 6 weeks before the certification expiration date. Registration and community credit verification forms, directions to sites, and screener status reports may be obtained from your screening coordinator. In compliance with UMDNJ - University Behavioral Healthcare - TAC policy, full TAC-CEU credits will be issued only to Screeners who attend the entire scheduled class. Partial credits will not be issued to Screeners arriving late or leaving early. All TAC Screener courses are approved for social work continuing education credits. *The University of Medicine & Dentistry reserves the right to cancel a training due to unforseen emergencies or low number of registrants and will not be held responsible for any expenses or problems that may occur as a result.
