Continuing Review Application

In an effort to streamline the continuing review process, the Human Subjects Protection Program office has revised the Continuing Review Application form.  This revised form is now available online. You may begin using this form immediately. 

Please note that the old Continuing Review Application form will not be accepted after September 1st.

The Protocol Reactivation Form has been replaced by the Expired Study Report Form.

Effective April 1, 2008, a new WIRB fee schedule will be put in place as
follows:

Initial Review - $1,750

Multicenter Studies - $825

Continuing Review - $825

Modifications - $ 285

Co-Investigator or Change in Investigator - $825
