HIV Medical Homes Resource Center (MHRC)
The François-Xavier Bagnoud Center of the School of Nursing at The University of Medicine and Dentistry of New Jersey, (FXBC/UMDNJ) and the University of California, San Francisco, Department of Family and Community Medicine, Center for Excellence in Primary Care (UCSF/CEPC) have collaborated to form the HIV Medical Homes Resource Center (HIV-MHRC), a Health Resources and Services Administration (HRSA) HIV/AIDS Bureau (HAB) funded national training and technical assistance cooperative agreement.
Characteristics of the Patient-Centered Medical Home (PC-MH) are team based care led by a personal physician or other eligible clinician, physician/clinician directed practice, whole person orientation through the provision of primary health care, integrated/coordinated care, quality and safety, enhanced patient access, and payment to reflect the added value to patients. The National HIV/AIDS Strategy (NHAS) emphasizes the need for a national effort to recruit and keep people living with HIV in care by establishing a seamless system to immediately link persons living with HIV/AIDS (PLWHA) to continuous and coordinated quality care. Building upon existing strengths of Ryan White HIV/AIDS Program grantees the HIV-MHRC provides an important resource to strengthen primary care for PLWHA and to improve health care access and outcomes for a population of adults and children at significant risk for health disparities.
The overarching goal of the HIV-MHRC is to provide training and technical assistance to assist Ryan White HIV/AIDS Program grantees in understanding, developing and successfully applying to become recognized PC-MHs.
The HIV-MHRC will accomplish this goal through the following activities:
- A cooperative agreement with HRSA/HAB
- PC-MH alignment with the NHAS as an approach for meeting the care needs of PLWHA
- Identification and linkages with existing national networks and resources
- Assessment of readiness and needs related to PC-MH development and certification
- Evaluation and dissemination of timely and available resources on PC-MH
- A comprehensive resource repository accessible on the HRSA TARGET Center website
- In-person strategic planning workshops with Ryan White grantees ready and interested in becoming a PC-MH
- Ongoing coaching, technical assistance and e-consultation led by a multidisciplinary team of practice facilitators
- A learning collaborative to share ideas, challenges, and innovations
- Webinar training series
- Documenting successful retention models of care
