INSTRUCTIONS FOR COMPLETING THE CHEMICAL INVENTORY FORM IN "EXCEL"

Chemical Inventory Form

When you open the form on your computer it will appear as an Excel file. If you click on the tab at the bottom, you can open up instructions, an example, and the actual form.

EOHSS is working with IST to develop a form that can be completed and submitted online. Until this format is available, when you are done completing the form, print out the tables and fax (973-972-3694) or mail them to Lan Nguyen, Senior Environmental Analyst, EOHSS,  65 Bergen Street (Bergen Building), Room 443, Newark Campus.  You may also email them to Lan (nguyenl1@umdnj.edu) as an attachment.

The Chemical Inventory Form must be updated annually.
