| To order air or rail tickets: |
| 1) |
Call agency for flight/rail information. |
| 2) |
Fax completed Travel Approval/Advance form to agency. |
| 3) |
Gateway will overnight tickets to requestor or traveler. |
| 4) |
Gateway will invoice University monthly based on tickets issued. Traveler does not have to use personal funds. |
| 5) |
Effective 9/1/10, all bookings will require the date of birth of the traveler. This rule was established by the Federal Transportaiton Security Administration. |
